What is the 1022 Club?

This beautiful, home-like facility is a place which promotes independence and wellness in older adults who may be frail, experiencing dementia or memory loss.

Who is Served by the 1022 Club?

Families caring for loved ones in their home are given the gift of time for work, errands, social activities & everyday life tasks. Knowing their loved one is safe & in a caring environment gives peace of mind to caregivers.

Services Provided

Services offered at the 1220 CLub may include, but are not limited to:

  • Medication supervision and administration
  • Family and physician updates if requested
  • Medically ordered diet and snacks
  • Evaluation by Adult Day Care Center activity personnel
  • Individually tailored recreational activities
  • Group activities with social interaction
  • Bath services with shampoo available
  • Provide a physical Therapy Maintenance Program
  • Flexible hours, Monday-Friday. Not available on holidays or weekend
  • Assistance in facilitating doctor or hospital appointments at health facility of the  family’s choosing
  • Education on disease and medications as needed to members and families
  • Referrals to other community resources to assist member and family as needs are identified
  • Threshold Adult Day Care does  not provide transportation to the Adult day Center, but coordinates with existing services
